Sometimes my patterns end up with skipped rows of stitching. I would think, ok, something's wrong with my machine / tension settings, etc. But these missed rows are kind of consistent when I repeatedly sew the same pattern. This makes me wonder if it's a glitch in my file. Any ideas appreciated.
That usually is a matter of fabric movement during the stitch out. So you may need to adjust stabilizer, better underlay and or lower stitch density to make it work better on the specific fabric.

You could also consider to break up large fill areas into smaller chunks of various stitch directions, if that's not breaking the look you're after.
